Our hours are Monday through Friday 8am-5pm and we see patients by appointment only. Please arrive 10-15 minutes before your scheduled appointment. If you are a new patient, have not been seen within the year, or your information has changed, please print and fill out the information update form and bring it to your appointment. Also, be sure to bring a photo ID and your insurance card with you. We understand that you may need to miss or cancel an appointment but we ask that you give us at least 24 hours notice.
We deliver and do gynecologic surgery exclusively at St. Francis Medical Center.

An update form needs to be completed if:
- this is your first visit
- you haven’t been seen within the year
- your insurance company or plan has changed
- your contact information has changed (name, address, phone, etc)

There will be a $15.00 fee for every FMLA, Disability or AFLAC form completed by your physician or any staff member. As a convenience, we request payment by cash or credit card at the time you drop off the paperwork. This allows us to mail or fax your forms when they are finished. We are happy to provide you with copies of your medical records. In order to obtain your records from another office we need your written authorization. You may use the release of records form below or fill out a copy in the office.
If you have billing questions call the office and ask to speak with our billing specialist. Please have your account number available when you call. Our billing specialist spends a large amount of time on the phone with insurance companies so you may be asked to leave a message. When leaving the message be sure to give detailed information including your name, account number, and contact information.
A HIPAA form will need to be completed on an annual basis to protect your confidentiality and the security of your healthcare information. These updated forms are kept in your electronic health record. You may change your preferences at any time by completing a new form.
